Applications must be submitted by 31 July 2024 at 5:00 PM

Please note that the grant is a limited fund, and the Local Authority will close the submission deadline earlier if the number of applications is likely to exceed the available funds.

BEFORE YOU APPLY

⇒ Applications are limited to one application per grant period

⇒ Maximum amount of the award for this period will be no more than £400
£75 for those already in receipt of other short breaks funded by the Local Authority

⇒ Ensure all unspent monies or proof of spends have been returned from any previous application

⇒ Have available image(s) of relevant supporting documents to upload

  • For child/young person with higher-level Disability Living Allowance (DLA)
    - image of front page of DLA award

  • For child/young person with Enhanced Personal Independence Payment (PIP)
    - image of front page of PIP award

  • For child/young person formally triaged or is current on Autism MAAT pathway
    - image of relevant EHCP assessment form or letter

  • For support care worker's time
    - image of relevant photographic evidence such as passport or photo driving licence

⇒ Each image must be a PDF, JPG or PNG and be no larger than 50MB in size

Stockton Borough Council uses your information in accordance with the Data Protection Act 2018 and the UK General Data Protection Regulation.

The purpose for collecting your data is to process the Short Break Grant application. The Council will process your information where there is a lawful basis to do so.

We will keep your information for a period of 18 months. After that time it will be deleted.
